IV Therapy
Units/hr to mL/hr (Heparin, Insulin) Calculator
Pump rate for infusions ordered in units per hour.
Formula
Concentration (units/mL) = units in bag ÷ bag mL
mL/hr = ordered units/hr ÷ concentration
How it is used
- Heparin and insulin are high-alert drugs: a second nurse verifies the calculation and the pump program.
- Weight-based heparin protocols give a bolus in units/kg and an infusion in units/kg/hr; multiply by the weight first.
